“We’re at a Complete Disconnect”: Marla Dubinsky, MD, on Bowel Urgency in IBD
In an interview with HCPLive at Digestive Disease Week (DDW) 2024 in Washington, DC, Marla Dubinsky, MD, chief of the division of pediatric gastroenterology at Mount Sinai Kravis Children’s Hospital, explains the disconnect between patients with Crohn disease and health care providers regarding the impact of and importance of addressing bowel urgency.
At the conference, she presented data from the Communicating Needs and Features of IBD Experiences (CONFIDE) study calling attention to these major communication gaps. Although US and European patients classified bowel urgency among the most reported and impactful symptoms, bowel urgency was not recognized by providers as a top symptom impact...

These are ridiculous accusations. To claim that half 1 million people die annually due to medical related errors is Stupidity. You can claim anything you want to buy twisting numbers. If you take a hospital fatality and review the entire medical record you will for sure. Find some error. That doesn’t mean the error caused the fatality. I’ve worked in healthcare as a surgeon for 30 years. Medical error related fatalities are really rare. Of course, as with anything it’s easy to play Monday morning quarterback. Personally, I only know of one fatality on a cancer patient due to an erroneously placed decimal point in chemotherapy dosing. This happened in the late 1980s. People seem to have the attitude that life is infinite and medicine should never have fatalities. What planet do you people live on? Claiming that half a million people die per year because of doctors or nurses who make mistakes it’s just stupid People with an agenda making fake news. What else is new.

I had a bad cold or flu last week and for 1.5 hours while I was trying to sleep on my back I was constantly coughing and I had a lot of gurgling and bubbling of fluid in my lungs. I've had that before so I was not alarmed but it was distressing to think I had the whole night ahead of me. I flipped over to sleep on my stomach (prone position) and ALL of the bubbling and gurgling in my lungs went away and the coughing reduced to near zero. I was shocked at how dramatic the change was. I was basically able to sleep the rest of the night. I also slept on my stomach the 2nd night, though I had a little bit of coughing but I was eventually able to sleep most of the night. To sleep on my stomach I sleep with my head on the edge of the pillow so my face is angled slightly down and I prop up my shoulder with 3 folded towels and I also lift my hip on that same side with a crumpled up bedsheet. This reduces the amount of neck rotation needed to sleep on my stomach. Another trick is to add something to keep your head from sliding off the pillow. It's hard to describe but basically I use pillowcase with the T-shirt in the end of it that sticks out from under my pillow and that creates a stop (like a wedge) to prevent my head from sliding off the side of the pillow. I've worked out these tricks for sleeping on my stomach over the past 2 years because I find stomach sleeping helps me sleep a lot better. I'm 57 years old and turning my head to the side to sleep on my stomach is more difficult than a younger person, so these methods reduce the amount the head needs to be turned for stomach sleeping.

Facilitate and make Available proper Scientific clinical trial research in Cerebrospinal Blood Flow Treatment/s, as Baseline Primary Care Option Treating Neurovascular Disease a mechanical issue NO drug, treatment, or therapy can solely rectify! What is the role of CCSVI Venous Hypertension and proper/improved Cerebrospinal Blood flow?? #CCSVI #BloodFlowMatters CCSVI is definitely one of the causes of MS. The novelty for some years is that we are certain that, after studying at the La Sapienza University of Rome, there are 3 types of CCSVI: * a 1 type with patients suffering from an obstacle to the endovacular venous discharge, i.e. due to congenital or acquired anomalies that restrict and block the drainage of the investigated veins (jugular, vertebral, azygos) * a 2nd type with patients suffering from an extra-vascular venous obstruction, i.e. due to external compression of the vessel * a 3 type with patients suffering from endo-vacular and extra-vascular venous obstruction So to simplify we can say that there is a CCSVI of the "hydraulic" type (1 type), a CCSVI of the "mechanical" type (2 type) and a "mixed" CCSVI of the two previous types (3 type), the 2 and the 3 type represent about 85% of cases. A patient with type 1 CCSVI will have a greater indication for angioplasty treatment, a patient with type 2 CCSVI will have a specific indication for specific physiotherapy decompressive treatment (RIMA Method), a patient with type 3 CCSVI it will have an indication for both an endo-vascular dilatation treatment and an extra-vascular decompression treatment. The RIMA Method devised by Dr. Domenico Ricci of Bari is able to release compressed veins throughout their course, as shown by a publication of June 2015 after a one-year study (Internal jugular Venous Compressione Syndrome: hemodynamic outcomes after cervical vertebral decompression manipulations-Pubmed). I've heard much more reputable psychologists, exercise physiologists, professional coaches, and neurologists counter what this fraud is saying. And I've read peer reviewed studies backing it up. It's basic psychology that people make actions to seek pleasure and avoid pain. People with eating disorders get more pleasure from food, so they need to get an equal or greater amount of pain and suffering from being obese. Fat shaming creates that, fat shaming motivates people to work harder. I had an eating disorder that causes me to have a really low appetite and I was bullied and skinny shamed for years and beaten up. Then I started busting my ass in the gym, lifting weights, running doing MMA etc. then I got better at fighting so I could fight back and forced fed myself for years. I was a 125 pound twig when I started and gained over 100 pounds of muscle. The military and military college also helped all the brutal training, hazing and the upperclassmen and NCOs targeting your insecurities and exploiting them. You won't accomplish anything without suffering and you won't accomplish anything great without great suffering it's a fact of nature. The problem with a lot of modern social scientists is they've never lived a real life, they've never really struggled, they haven't suffered in the real world. They've lived soft coddled lives and have wealthy careers that involve little to no stress and operate under this mentality that problems should be tried to cure without any pain. But pain builds character and inner strength. This is proven, that's why every culture since the beginning of time has had hazing, and initiation rituals. People used to be put through hell to be given the strength and discipline to endure the struggles of life. Holding people's hands and coddling them just enables their self destructive behavior. You are just enabling it by being soft on them. I'm a living example of how negative motivators work. I've also lost a ton of weight in short periods of time. I've fasted foe 7 days straight and I do a 36-48 hour fast every week. It sucks but it's more than possible. The average human can go 30 days without food. The SOLE reason for obesity is laziness. Or you could say laziness and gluttony. You can try to excuse the gluttony by calling it an eating disorder but gluttony is gluttony it's the same for everyone. You get a dopamine kick from food and feel stress in the absence of food, sure that's a proven fact. But who's to say you have the right to give in to impulses for pleasure or to avoid pain out of fear. No matter the reason for the impulse, if you give into to then it's still YOUR fault. You could psychologically explain it all away and show the exact neurological reason why it happened but if you apply that logic then you could do the same for rapists and pedophiles. Everyone has a psychological reason foe bad behavior but as a society we can't just simply accept bad behavior just because we know the reason. Technically the science shows us that free will doesn't exist at all, everything is a result of cause and effect subject to the laws of nature. But a culture constructed on the belief in free will and personality is always superior to one that accepts that we have no free will, even if it's technically true. It's also a semantics issue, depends on how you define free will. But we know that we want people to believe they have free will and that they are responsible for their actions, we all inherently accept this and we raise our children on this principle, we punish criminals on this principle. We can't play both sides. Do we want to accept free will or reject it?

Fat shaming is a proven solution to obesity. It worked for generations, I've seen it work. Obese people don't need treatment they need diet and exercise. It's calories in and calories out. It's very simple.
